How FORMery collects, uses and protects your personal information.

What we collect
- When you book a session: name, contact details, payment information and any health or movement information you share with us. Bookings are processed through Trafft; see “Who we share it with” below.
- When you make a corporate enquiry: your name, role, contact details, organisation and what you tell us about your objectives.
- When you apply to the Collective: name, contact details, location, qualifications, insurance status, DBS status, experience, availability and anything else you include.
- When you join a mailing list or waitlist: name, email and any preferences you give us.

Health information
